What is the difference between primary and secondary breast reconstruction?

Primary (immediate) reconstruction begins during the same operation as the mastectomy or lumpectomy, so you wake with the reconstruction already underway. Secondary (delayed) reconstruction takes place months or years later, after cancer treatment is complete and the tissues have healed. Immediate reconstruction means one fewer major operation and preserves more of your own skin; delayed reconstruction gives you time to research options and avoids exposing a fresh reconstruction to radiotherapy. Reconstruction remains possible years, even decades, after a mastectomy.

How does radiotherapy affect breast reconstruction?

Radiotherapy is the single strongest influence on reconstruction timing. Radiotherapy given after a reconstruction can cause firmness or capsular contracture around implants, fat necrosis in flap reconstructions, changes in size and shape, and wound healing problems. Where radiotherapy is anticipated, the standard approach is either to delay reconstruction until it is complete, or to use your own tissue rather than an implant. Combining a tissue expander with radiotherapy carries up to a 25% complication risk, and capsular contracture runs at 40–50% in irradiated implants compared with 10–15% without radiotherapy.

How long do I have to wait after radiotherapy before breast reconstruction?

Most surgeons advise waiting 6 to 12 months after radiotherapy ends before delayed reconstruction, so tissues can stabilise and inflammation resolve. Radiation effects pass through an acute inflammatory phase during treatment, a fibrotic phase over the following months, and a late remodelling phase; some changes continue evolving for 1 to 2 years. Waiting significantly reduces complications.

How long after chemotherapy can breast reconstruction be performed?

Chemotherapy temporarily reduces immune function and healing capacity, so major reconstruction ideally takes place either before chemotherapy begins or several weeks after it finishes. Reported waiting periods range from 4 weeks to 3 months; in the Netherlands, for example, surgeons prefer to wait at least 3 months after the last chemotherapy before an elective procedure. Some women have a mastectomy with immediate tissue expander placement — a shorter procedure — and complete the reconstruction after chemotherapy.

What breast reconstruction techniques are there?

After a mastectomy the main routes are: no reconstruction with an optional external prosthesis; tissue-conserving reconstruction such as the Goldilocks procedure; implant-based reconstruction, either two-stage with a tissue expander or direct-to-implant; and reconstruction with your own tissue — a DIEP flap from the abdomen, a latissimus dorsi flap from the back, gluteal flaps from the buttock, thigh flaps, or total reconstruction by staged fat grafting. After breast-conserving surgery the options are oncoplastic reshaping techniques, volume replacement flaps such as LICAP and AICAP, and later correction with fat grafting or symmetry surgery.
